EnergySolutions Foundation Scholarship

Transcript:
All applicants must include a high school transcript with grades from 9th grade and grades from the first half of your 10th grade year. If in any class grades are not given until the end of the school year, please ask your teacher to give a grade for the first half of the year. Have transcripts completed by the appropriate school official. DO NOT INCLUDE SOCIAL SECURITY NUMBERS. Online transcripts/grade reports are not accepted. You must also include a copy of your class schedule for the remainder of your sophomore year. If these requirements are not met, you will not be considered for the scholarship.
Deadline is February 3, 2010. Transcript/Grades may be mailed to PO Box 510583, SLC, Utah 84151, faxed to 801-413-5697, or emailed to msmannebach@energysolutionsfoundation.org.
It is your, the student's, responsibility to check with the school to make sure your transcripts are sent before the deadline. No Late Transcripts Accepted. Transcripts can be sent by the school or by the student. DO NOT INCLUDE SOCIAL SECURITY NUMBERS ON TRANSCRIPTS.
Recommendations:
To the Applicant: Two recommendation letters are required. If incomplete, you will not be considered for the scholarship. This section is to be completed by a teacher, counselor, advisor or an instructor who knows you well. More points will be given to recommendations from math, science and engineering teachers. Recommendations may be submitted by downloading the online form and sending the recommendations separately from the application or by having recommenders follow the link below to complete the form online. Deadline is February 3, 2010.
It is your responsibility to check with your recommenders to ensure that your recommendations are sent by the deadline. These are to be submitted by the recommender or given to the student in a sealed envelope to be submitted with his/her application. No Late Recommendations Accepted.
Application:
The online application has three separate pages. It is recommended you first write your essay in a Word document before filling out the application online. Once the essay is written, you will be ready to open and complete the application in one sitting. When you hit the submit button your information will be sent to the EnergySolutions Foundation. When transmission is successful, you will automatically receive a copy of your application via email.
For an application to be considered complete, the following material must be received by the EnergySolutions Foundation:
- __ Application filled out by 10th grade student.
- __ Transcript(s) for 9th and 10th grade. (Online transcripts are not acceptable.)
- __ Complete copy of 10th grade schedule.
- __ Essay. No handwritten essays accepted.
- __ Two recommendations
- __ You have indicated an approved major of study.

Special notes for completing online application:
- Applicants must have Adobe Acrobat Reader to complete the application online. If necessary, download the most recent version.
- Applicants must complete the application in one sitting; data will not be automatically saved.
- It is recommended that you print the application before hitting submit. Remember data does not get saved. (When complete and while the document is open, click on the File menu, choose Print, and click OK. Once the document prints, click Submit.)
- After submitting, you will receive a confirmation email which includes a copy of your application. This email confirms that your application was submitted successfully. If you receive this confirmation there is no need to print and mail in a copy to the Foundation. If you DO NOT receive this email, we did not receive your application. In this case you can either mail in a copy of your printed application or resubmit online.
- If you send in the essay separatly from the rest of the application, it's imperative that you include your state, school, name, and title of the essay. If your essay is not identified properly, your essay will not be matched up with your application and therefore your application will be considered incomplete.
Important Reminder!
Before leaving the application site or hitting submit, print the application for your records!
Data will not be saved.
The student is responsible for contacting the EnergySolutions Foundation to ensure that all materials were received.
